Composition :

Each vial GOFORAN® contain Cefotaxime Sodium equivalent with Cefotaxime 1.0 g

 

Pharmacology :

GOFORAN® with active ingredients Cefotaxime belongs to a third generation cephalosporin antibiotic that has anti-bacterial activity by inhibiting the synthesis of mucopeptides in the bacterial cell wall and has broad activity against gram-positive and gram-negative organisms. Cefotaxime has high stability to beta-lactamase, either penicillinase or cephalosporinase produced by gram negative and gram-positive bacteria. Also shows the inhibitory ability of beta-lactamase production in gram-negative bacteria.

 

Indication :

GOFORAN® effective for the treatment of infections caused by sensitive microorganisms, as in:

  • Lower respiratory tract infections, including pneumonia caused by S. pneumoniae, S. aureus (producing and non-penicillinase producing), E. coli, Klebsiella spp, H. influenzae (including strains ampicillin-resistant), P. mirabilis, S marcescens and Enterobacter spp.
  • Urinary tract infections caused by Enterococcus spp, S. epidermidis, Citrobacter spp, Enterobacter spp, E. coli, Klebsiella spp, P. mirabilis, S. marcescens, uncomplicated gonorrhea caused by N. gonorrhoeae also (including strains producing penicillinase).
  • Gynecological infections include pelvic inflammation, endometritis caused by S. epidermidis, Streptococcus spp, Enterococcus spp, E. coli, P. mirabilis, Bacteroides spp, Clostridium spp, and anaerobic cocci (including Peptostreptococcus spp and Peptococcus spp).
  • Septicemia caused by E. coli, Klebsiella spp and S. marcescens.
  • Central nervous system infections, such as meningitis and ventriculitis caused by Neisseria meningitidis, H. influenzae, S. pneumonia, Klebsiella pneumoniae and E. coli.
  • Skin and soft tissue infections caused by S. aureus, S. epidermidis, S. pyogenes, Enterococcus spp, E. coli, Enterobacter spp, Klebsiella spp, P. mirabilis, Pseudomonas spp, S. marcescens, Bacteroides spp and anaerobic coccus.
  • Intra-abdominal infections, including peritonitis caused by E. coli, Klebsiella spp, Bacteroides spp and anaerobic coccus.
  • Bone and or joint infections caused by S. aureus (producer and non-penicillinase producer), Streptococcus spp, Pseudomonas spp and P. mirabilis.
  • Prevention of postoperative infections.

 

Dosage :

Adult

  • Usually dose: 1 g every 6-8 hours, maximum dose 12 g daily.
  • Mild infection: 1 g every 12 hours.
  • Moderate to severe infections: 1-2 g every 6-8 hours (i.m or i.v).
  • Life-threatening infection: 2 g every 4 hours (i.v).
  • Gonorrhea: 0.5 – 1 g every 8 hours or a single dose.
  • Rectal gonorrhoe in women: 0.5 g i.m (single dose), in men 1 g i.m (single dos).
  • Infection requiring high dose antibiotic (septicemia): 2 g every 6-8 hours (i.v).
  • Gonococcal urethritis / cervicitis in women or men: 0.5 gram i.m (single dose).
  • Prevention of postoperative infection: single dose of 1 g (i.m or i.v) given 30-90 minutes before starting the surgery.

 

Children

  • Infants less than 1 week: 50 mg / kg BW every 12 hours (i.v).
  • Infants 1-4 weeks: 50 mg / kg BW every 8 hours (i.v).
  • Children 1 month – 12 years old: 50-180 mg / kg body weight daily in divided doses 4-6 times (i.m or i.v).
  • Children weighing more than 50 kg equals adult dose (maximum 12 g / day).

 

In patients with renal impairment:

Patients with ‘creatinine clearance’ less than 20 ml / min / 1.73 m2: dose reduced by half.

 

Infection caused by Streptococcus haemoliticus:

Treatment is given for at least 10 days to prevent the risk of rheumatic or glomerulonephritis.

 

Preparation of Injection Solution :

I.V administration

I.V Intermitten: For 1.0 g solution in 10 ml can be injected for 3-5 minutes

I.V Infus         : Reconstitution with 50 ml or 100 ml of 0.9% NaCl injection or injection

of dextrose 5%, then infused for 20-60 minutes

I.M administration

Injectable in the gluteus maximus, sholud not more than 4 ml on each side. Pain can be avoided by dissolving in 1% lidocaine solution.
