Composition :

Each 5 mL (1 scoop) of GARKENE® syrup contains 250 mg of Valproic acid.

 

Pharmacology :

Valproic acid undergoes dissociation (breakdown) into Valproate ion in the digestive tract. The mechanism of action of Valproic acid as an antiepileptic is not known with certainty and is thought to be related to an increase in the concentration of gamma-aminobutyric acid (GABA) in the brain.

 

Indications:

Used as a single or adjunct therapy in the treatment of partial seizures (simple and complex) and absence seizures (petit mal).

Dosage:

Given orally. The recommended starting dose is 15 mg/kg/day, increasing at 1-week intervals of 5-10 mg/kg/day, until seizures are controlled or side effects do not become more severe. The maximum recommended dose is 60 mg/kg/day. If the total daily dose exceeds 250 mg, Valproic acid should be given in divided doses.

The frequency of occurrence of side effects (especially elevated liver enzymes) may be dose related. The benefits of increased seizure control that may accompany an increase in dose must be weighed against the greater likelihood of adverse events.

There has been no finding of a good correlation between daily dose, serum levels, and therapeutic effect. However therapeutic serum levels for most patients will range from 50 – 100 mcg/mL. Occasionally patients can be controlled with serum levels of phenobarbital and/or phenytoin.

Patients with gastrointestinal irritation can take the drug with food or by starting the drug from a low initial dose.
